NEMT Setup Certainty™ | NEMT Self-Pay Setup Framework

Set Up Your NEMT Company to Get Paid Directly — Instead of Waiting on a Broker.

NEMT Setup Certainty framework shown across desktop and laptop screens with its lessons and bonus resources.

Nine short lessons, your actual documents, one clear answer: can this company take a self-pay client as it’s built right now? You’ll know what you have, what it supports, and what your next decision should be — without refiling anything or starting over.

Your actual setup. One clear decision about self-pay.

Show Me If I Can Take Self-Pay

By the Time You’re Done, You’ll Know:

CAN MY SETUP SUPPORT SELF-PAY?

A straight answer from your own filings — not a general explanation of how NEMT works. Yes, no, or not yet.

WHERE DOES MY SETUP POINT?

Your business name/entity setup, EIN, NPI, and D-U-N-S profile. You filed them. This is where you read them together and see which direction they’re pointing.

WHAT NEEDS MY ATTENTION FIRST?

Most of what you have is probably fine. You’ll know which one piece deserves a closer look — before you refile, rename, or pay anybody to fix something that was never broken.

WHAT SHOULD I DO NEXT?

Stay with the build you have, start moving toward self-pay, or verify one thing first. You leave with the decision made, not a list of questions.

NEMT Setup Certainty framework and four-document resources.

The van payment doesn’t pause while you figure this out.

Show Me If I Can Take Self-Pay

I Kept Meeting the Same Person.

Here is the NEMT owner I keep hearing from:

Coach Willena McGee seated and smiling.

They followed the NEMT checklist they were given. Form the LLC. Get the EIN. Buy the vehicle. Get insured. Apply with the brokers.

At the time, that broker-first model made sense. Broker volume was lucrative, and it was the business model most people in the industry were teaching.

Then the contracting slowed.

Some owners are being told the broker wait could be six to twelve months.

The van payment did not stop. The insurance did not stop. The operating expenses did not stop.

So now the question I hear is different:

“Coach, how do I pivot to self-pay?”

And that is when I ask a different question: What are you actually building?

Because the paperwork was never just a stack of boxes to check. It was describing a business model. If the next move is self-pay, you need to understand what your current records support before you build on top of them.

THIS FRAMEWORK IS PROBABLY FOR YOU IF...

You already bought the van.

The insurance is active. The operating expenses are showing up every month. And now the broker is telling you contracting could take six to twelve months. So the question has changed from “When will the trips start?” to “How do I pivot to self-pay?”

You did exactly what the NEMT influencers told you to do.

LLC. EIN. Vehicle. Insurance. Apply with the brokers. Check, check, check, check. But nobody explained what that checklist was actually building — or what that could mean if you later wanted to operate differently.

You are ready to move toward self-pay.

But before you start changing paperwork, adding services, or following another checklist, you want to evaluate what you already have and understand whether your current setup supports where you are trying to go next.

You have not made the expensive move yet.

Maybe the van is not sitting in the driveway yet. Maybe you have not committed to the insurance, equipment, or another round of filings. You would rather understand the business you are building before you spend thousands of dollars building on top of it.

You Probably Followed the Influencers’ Checklist.

And that checklist probably looked something like this:

  • “Get your LLC.” — Check.
  • “Get your EIN.” — Check.
  • “Buy the van.” — Check.
  • “Get insured.” — Check.
  • “Apply with the brokers.” — Check.
  • “Now wait for trips.”

The problem isn’t that you followed the checklist. The problem is nobody told you what that checklist was building.

When broker volume was flowing, you probably didn’t have a reason to question it.

But now brokers aren’t contracting like they used to, some owners are being told the wait could be six to twelve months, and suddenly “just wait for trips” isn’t much of a business strategy.

So now you’re asking, “How do I pivot to self-pay?”

Before you change your name, your codes, your paperwork, your services—or pay somebody else to “fix” something—look at what those original choices actually built.

Broker-first, built with self-pay in mind, or somewhere in between? That’s the question the NEMT Setup Certainty™ Framework helps you answer.

They Chose the Self-Pay Business Model First.

One of my recent coaching graduates, Merit Care Enterprises in Baltimore, Maryland, did it differently.

They chose the self-pay business model first instead of building the company around one payer path.

Within 90 days of launch, Merit Care sold their first $800 self-pay package.

They knew what they were building before they built around it.

“Very patient and caring! Great to work with. Love that the drivers are registered nurses!”

I’m Coach Willena McGee, RN, BSN.

After shutting down J & L’s Medical Transport, I transformed what I learned from that experience to become the WM NEMT Startup Coach®. I know what it costs to build from a checklist without understanding the business underneath it.

I built J & L’s Medical Transport, made expensive decisions before the foundation was clear, and eventually shut the company down. At the time, I was following the same broker-first checklist that had been given to me by someone who was already in the business. I did not know there was another way to think about the business either.

Today, I bring more than twenty years of experience as a registered nurse to this work, and I am an AAA-accredited training provider, TP24831.

I am creating CEOs—not just providers. That means teaching NEMT entrepreneurs to critically think about the companies they are building, understand their own records, and make decisions from strategy instead of dependency.

Before you build the next layer, understand the one underneath it.

Your NEMT Self-Pay Pivot Shouldn’t Start With Another Checklist.

It should start with knowing whether your current setup can support self-pay.

Pull your records. See what they support. Know whether your setup leans broker-first, supports self-pay, or needs another look — then decide what comes next.

NEMT Setup Certainty™ gives you the framework to do exactly that.
